Types of data I process
In order to contact you and work with you safely, I need certain data about you.
I need to be able to contact you, address you and arrange appointments with you. This includes your name, address, telephone and email address. I may use third party software, for example to facilitate the arranging of appointments and to take payment.
I need some contact details in case of emergency. I will use these if I have concerns for your safety.
I will make brief notes about your sessions with me.
I may ask you to complete outcome measures, usually in the form of questionnaires. These measures are designed to measure movement towards your goals or quality of wellbeing that result from the work we do together. I will store your individual response as part of your notes but I may also use the outcome scores to compile aggregate data on the changes experienced by my clients.
Sharing policy
I will never give your details to a third party except in cases where you give your explicit consent or if I am compelled by the law.
To work safely, I must discuss my cases and way of working with other, suitably qualified professionals – this is called Supervision. All such discussion is anonymised so that your name and identifying details are not mentioned in these discussions.
Data retention
I keep your data and notes on file for seven years, and then they are destroyed. If my client is a minor, the records will be kept for seven years after they have turned 18. Aggregate data may be kept indefinitely.
Data subject rights
My use of data is limited to what I need to do my job. If you have any concerns, I encourage you to contact me to discuss them. More information on your rights can be found on the ICO website
